Immediate Loading or a “Regular” Implant — How to Decide

The difference between the protocols is not the implant but the timing of the restoration: with immediate loading a fixed provisional bridge is attached the same day; with delayed loading the implants heal for 3–6 months before anything touches them. The deciding factor is primary stability — how firmly the implant “grips” the bone on the day it is placed.

What actually happens in the bone

An implant bonds to bone through osseointegration: bone cells grow onto the titanium and lock it in. In the first weeks the initial mechanical hold (threads in bone) weakens while the biological bond is only beginning — the “vulnerable window”. Movement of the implant in that window can produce scar tissue instead of bone. So the only question that matters is: can we guarantee the implant will not move?

How immediate loading solves it

Two mechanisms. First, high primary stability: an implant measured above a certain threshold (roughly 35 Ncm of insertion torque, or a matching ISQ value) holds. Second, cross-arch rigidity: when four or six implants are joined by one rigid bridge, a force on one is shared by all, and none moves alone. That is why immediate loading of a full arch is safer than immediate loading of a single implant in a chewing zone.

When we prefer to wait

  • Low primary stability on the day of surgery — measured, not guessed
  • A single implant in the molar region with high chewing forces
  • After a large bone graft or an open sinus lift
  • A patient who cannot keep to a soft diet

The final decision is always made twice: once in planning on the CT, and once on the day of surgery with the stability meter in hand. Anyone who promises “teeth in a day” before measuring is promising something they do not yet know.
